Leave of Absence
Leave of absence requests must be directed to Client Services office via telephone or in person, at which point official form must be completed and signed. For convenience, a faxed signature is acceptable on pre-authorized form. We do not accept phone-only requests, email-only requests, or anyone other than the member’s signature; unless for reasons of member’s disability.
- Members may request a Leave of Absence in month-long blocks of one, two, or three months; an automatic reactivation date is required at the time of request.
- A member may be on a leave of absence for no more than three months of any twelve, except in the case of an extended disability/illness.
- The fee for monthly members is $10 per month, paid in advance.
- Leave of Absence request requires 30 days notice prior to billing date.
- Monthly billing will continue as scheduled according to the terms of your Membership Agreement.
- Time owed per membership agreement will be added to the end of your initial term membership.
- A Leave of Absence may not be retroactive, and may not be reinstated.
- While on Leave of Absence, members may not use the club as a Guest, and member-privilege discounts are not available
- For Medical Leaves of Absence:
- Members may request a Medical Leave of Absence for VALID and documented MEDICAL reasons. Documentation must be completed by attending physician, and returned to the club at time of injury/illness
- In the event of an extended disability/illness, a new Medical Leave of Absence Form must be submitted every three months or membership will be reinstated.
Cancellation
